Bergen County Prosecutor Mark Musella introduced the arrest of CARMEN
BALLESTAS (DOB: 08/17/1959; married; unemployed) of 251 Sutton Avenue,
Hackensack, NJ, on costs of violating a provision of the New Jersey Insurance coverage Fraud
Prevention Act. The arrest is the results of an investigation carried out by the Bergen
County Prosecutor’s Workplace underneath the path of Chief Robert Anzilotti and the Nationwide
Insurance coverage Crime Bureau underneath the path of Mid-Atlantic Regional Director Gary Healy.

On or round October 2018, the Bergen County Prosecutor’s Workplace Monetary
